We provide support for patients admitted at Frimley Park Hospital, Wexham Park Hospital and Farnham Hospital.
Outpatient / community services
We run a variety of outpatient clinics, webinars and courses covering a wide range of conditions. These may take place via telephone / virtual appointments or face to face at locations across the local area. We also provide support for housebound individuals at risk of malnutrition, and patients who are tube fed at home.
