The Integrated Frailty Liaison teams at Frimley Park and Wexham Park hospitals are innovative multi-disciplinary teams, providing a specialised service for patients living with frailty who attend hospital.
Their aim is ‘to develop, create and deliver a person-centred pathway that supports those patients living with frailty to access the highest quality acute care in the right place at the right time, while maximising independence and reducing length of stay in hospital.’
The teams are comprised of a mix of Frailty Practitioners, including doctors, in-reach General Practitioners (GPs), nurses, physiotherapists, occupational therapists and pharmacists, supported by consultant geriatricians. They see people who attend the Emergency Department with conditions associated with frailty, and also inpatients on hospital wards who are referred to them for a specialist frailty assessment.
The frailty teams provide standardised comprehensive geriatric assessment for patients referred to them, and they work very closely with community-based teams. This enables them to assess people with frailty in a truly holistic way, and to help bridge the gap for patients between hospital and home.
