Are you concerned about a patient’s clinical condition?
We recognise friends and family can often see a patient’s deterioration before anyone else does.
If you still have concerns after talking to the medical team, you can contact the critical care outreach team directly.
- At Frimley Park Hospital, call 07768 131445
- At Wexham Park Hospital, call 07887 292886
If appropriate the Trust’s critical care outreach practitioners will review the patient, liaise with the clinical team, and take any appropriate immediate action.

What is Call 4 Concern?
The Call 4 Concern© programme enables friends, relatives – and patients themselves- to make a direct referral to the critical care outreach team if they feel the clinical condition of an adult or child in-patient is actively deteriorating.
They should have spoken to the ward nursing and clinical teams in the first instance.
The programme was created by Mandy Odell, consultant nurse in critical care, at Royal Berkshire NHS Foundation Trust.
Outreach practitioner Clare Ratcliffe said: “We know that our patients’ loved ones know them best and can sometimes be the first to recognise subtle changes in their condition.
“The NHS already has robust systems and processes to detect early signs of deterioration in patients and Call 4 Concern is another layer of reassurance for our patients and their families.”
The Call 4 Concern programme is available 24 hours a day, seven days a week and has previously been successfully implemented by several other NHS organisations.
Users of Call 4 Concern have provided feedback that has helped shape service changes. If you have used the service we'd really like to hear from you- please complete our short survey.
